Responsibilities
- Collaborate with interdisciplinary teams to develop and implement case management plans
- Assess patients' health needs and develop care plans
- Coordinate care delivery across various settings
- Monitor patient progress and adjust care plans as necessary
- Communicate effectively with patients, families, and other healthcare providers
- Ensure compliance with regulatory standards and policies
Requirements
- RN with a minimum of 1 year in specialty
- 6 months of experience in Allied field
- Adventurous spirit and fierce dedication
- Appropriate certifications and licensure as needed for the specific position
