Travel PT Home Health - $2,406 per week

Job Description

Ardor Health Solutions is seeking a travel Home Health Physical Therapist for a 13-week assignment in Lake Zurich, Illinois.

Requirements

• Graduate of an accredited Physical Therapy program
• Current Illinois PT license or ability to obtain prior to start
• Current BLS certification
• Previous Home Health experience preferred
• OASIS experience preferred
• Valid driver's license and reliable transportation
• Comfortable independently managing a Home Health territory

Qualifications

• Comfortable independently managing a Home Health territory
• Coverage Area & Mileage: Larger territory covering portions of the northern and northwestern Chicago suburbs. Coverage can extend from areas such as Evanston and Highland Park west toward Marengo. Travel is estimated at 200+ miles per week.

What You'll Do

• Complete PT evaluations and reassessments in patients' homes
• Develop individualized plans of care
• Provide gait, balance, strengthening, transfer, and mobility training
• Treat orthopedic, neurological, post-surgical, and medically complex patients
• Provide fall-prevention and home-safety education
• Educate patients and caregivers
• Collaborate with the interdisciplinary Home Health team
• Complete timely clinical documentation
