Travel PT Home Health - $2,435 per week

About the role

We are seeking a dedicated Home Health Registered Nurse to provide high-quality patient care in a home setting. The ideal candidate will have experience in skilled home care and be able to perform various assessments and interventions to promote recovery and rehabilitation of patients.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and their families on care protocols and wellness strategies.
• Monitor vital signs and report changes in patient conditions.
• Utilize adaptive devices and assistive technologies to improve patient mobility and safety.
• Document patient progress and maintain accurate medical records.
• Coordinate with interdisciplinary teams to optimize patient care.

Requirements

• Minimum of 1 year of home health experience required.
• BLS certification is mandatory.
• State nursing license or compact license required.
• OASIS training preferred.
• Experience with electronic charting systems (Cerner preferred).
• Valid driver’s license and reliable transportation for home visits.
• COVID-19 vaccination required or valid exemption.
• Flu vaccination required or valid declination with mandatory mask wearing during work hours.
