Physical Therapy Assistant – Home Health (Travel)

Location: Monterey, California

Start Date: 08/24/2026

Contract Duration: 13 weeks

Shift: Day Shift

Weekly Pay Range: $1,199–$1,261

Responsibilities

• Provide skilled PTA services under supervision per the plan of care for residents in the facility
• Implement and progress therapeutic exercises, gait training, transfers, and modality use as directed by the supervising PT
• Maintain accurate documentation in the patient record, ensuring accuracy and compliance with regulatory standards
• Monitor patient response, document progress, and adjust treatment as authorized; communicate changes with the supervising PT
• Auxiliary patient education, safety, and discharge planning in collaboration with the interdisciplinary team

Requirements

• Active California Physical Therapy Assistant license
• SNF experience preferred
• NetHealth (Rehab Optima) knowledge preferred
• Knowledge of home health concepts is beneficial given the specialty focus
• Ability to work 38 hours per week on day shifts
• Willingness to travel and work in a skilled nursing facility setting
