Travel Physical Therapist - SNF / LTC - $2,400 per week

Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Anderson, Indiana.

About the role

A great SNF / LTC facility is looking for an experienced or newly graduated, hard-working Physical Therapist to join their growing team. The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services, including:

• Providing direct patient care to assess medical condition, functional capabilities, limitations, and restrictions
• Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician
• Setting realistic and achievable goals for patients
• Documenting and recording the patient's condition
• Educating patients and families in appropriate physical therapy methods

Responsibilities

• Develop a plan of care for each physical therapy patient
• Provide skilled physical therapy services/interventions in accordance with physician orders
• Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions
• Collaborate with all disciplines to plan and evaluate team goals for each patient

Requirements

• Must have graduated from an accredited school
• Current state license as a Registered Physical Therapist or proactively in the process of obtaining licensure
• Current CPR certification

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
