About the role

Physical Therapist needed for a Skilled Nursing Facility in Nashua, New Hampshire, United States. Start date 08/31/2026 for a 13-week travel assignment. Weekly pay range $2,463–$2,597 per week with guaranteed 40 hours. Shift is 5×8 days, 08:00–16:30. Setting: long-term care / skilled nursing facility. Travel assignment through Healthforce Healthcare LLC.

Responsibilities

• Conduct initial and ongoing PT evaluations to determine resident needs and goals
• Develop and implement individualized therapy programs to improve mobility, strength, balance, and functional independence
• Reassess progress, adjust treatment plans, and set measurable outcomes in collaboration with the interdisciplinary team
• Document evaluations, treatment plans, progress notes, and outcomes in accordance with regulatory and facility standards
• Provide patient and caregiver education, safety training, and fall-prevention strategies
• Collaborate with nursing, medical staff, and other therapy disciplines to coordinate comprehensive resident care

Requirements

• Active Physical Therapist license in New Hampshire or eligible for licensure in NH
• Ability to practice in a skilled nursing facility setting
• Strong documentation and communication skills; comfortable working with a multidisciplinary team
• Available for a 13-week travel assignment with a 40-hour work week
