Responsibilities

• Conduct initial and ongoing physical therapy evaluations for residents in a skilled nursing setting.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Administer evidence‑based interventions and document patient progress, updating plans as needed.
• Communicate and collaborate with the interdisciplinary team to coordinate care and support discharge planning.
• Educate residents and families on home exercise programs, safety, and strategies to maintain gains.

Requirements

• Active physical therapy license in the United States with eligibility to practice in Washington State.
• Current CPR/BLS certification.
• 1+ year of physical therapy experience; SNF or long‑term care experience preferred.
• Graduate of an accredited physical therapy program.
• Ability to work effectively in a skilled nursing facility environment and participate in a temporary travel assignment.
