Travel Physical Therapist Skilled Nursing Facility Job

Physical Therapist needed for a travel assignment in a skilled nursing facility located in Franconia, New Hampshire. Start date 10/19/2026 for a 13-week engagement with guaranteed 36 hours per week. Responsibilities

• Conduct comprehensive PT evaluations and develop individualized plans of care for residents
• Implement evidence-based PT interventions to improve mobility, strength, balance, and gait
• Monitor progress, adjust treatment plans as needed, and document outcomes accurately
• Collaborate with the interdisciplinary team to coordinate goals, progress, and discharge planning
• Educate residents and families on therapy plans, safety, and home exercise programs
• Ensure compliance with regulatory and facility policies, including documentation standards

Requirements

• Ability to work effectively in a skilled nursing facility setting
• Strong clinical assessment, documentation, and time-management skills
• Excellent communication with patients, families, and care teams
