About the role
Care Team Solutions is seeking a travel Long Term Care (LTC) Physical Therapist for a 13-week assignment starting July 27, 2026, in Ada, Oklahoma.
Responsibilities
- Provide physical therapy services to residents in long-term care facilities.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patients' functional abilities and modify treatment plans accordingly.
- Document all patient interactions and progress in accordance with facility policies.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
- Current licensure as a Physical Therapist in the state of Oklahoma.
- At least 2+ years of recent experience in Long Term Care settings.
- Knowledge of Long Term Care regulations and standards.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
Qualifications
- Graduation from an accredited Physical Therapy program.
- Passing scores on the National Physical Therapy Examination (NPTE).
- Current CPR certification.
Skills
- Strong clinical skills in physical therapy.
- Effective communication and teamwork abilities.
- Ability to manage time effectively and prioritize tasks.
