About the role
Care Team Solutions is seeking a travel Long Term Care (LTC) Physical Therapist for a 13-week assignment starting July 27, 2026, in Moore, Oklahoma.
Responsibilities
- Provide physical therapy services to residents in long-term care facilities.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patients' functional abilities and develop appropriate exercise programs.
- Document all treatments and progress in accordance with facility policies and regulations.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
- Current licensure as a Physical Therapist in Oklahoma.
- At least 2+ years of recent experience in Long Term Care.
- Knowledge of long-term care physical therapy practices and protocols.
- Ability to work independently and as part of a team.
- Valid driver’s license and reliable transportation.
Qualifications
- Graduation from an accredited Physical Therapy program.
- Passing scores on the National Physical Therapy Examination (NPTE).
- Current CPR certification.

Gross Weekly Pay: $2205.25
Schedule
