Travel Physical Therapist - Long Term Care - $2,450 per week

About the Role

MetaSense, Inc. is seeking a travel Long Term Care (LTC) Physical Therapist for an assignment in Moore, Oklahoma. This is a 6-week travel position starting 09/14/2026, with a schedule of 40 hours per week, 8-hour day shifts.

Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs.
• Develop individualized plans of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practices.
• Provide manual techniques, ambulation training, therapeutic exercises, modalities, supportive/assistive devices, wound care, and other appropriate interventions.
• Monitor patient progress and modify treatment plans based on clinical outcomes.
• Evaluate treatment effectiveness and make referrals when appropriate.
• Provide consultation and education to patients, families, caregivers, and other healthcare providers.
• Communicate effectively with supervisors and interdisciplinary team members regarding patient progress, barriers, and treatment plans.
• Complete comprehensive discharge summaries and maintain accurate electronic documentation.
• Accurately record treatment minutes in accordance with regulatory and clinical requirements.
• Participate in patient care conferences, utilization review meetings, rehabilitation meetings, trainings, in-services, and other required activities.
• Obtain necessary DME and supplies as needed.
• Supervise and support Physical Therapist Assistants, rehabilitation aides, and/or students.
• Practice in accordance with APTA standards, Oklahoma regulations, professional ethics, and applicable practice requirements.

Requirements

• 1+ year of Physical Therapist experience (Required).
• Long Term Care (LTC) or Rehab experience (Required).
• Active Oklahoma PT license (Required).
• New graduates are welcome with Level II clinical experience; Level I observation-only experience does not qualify.

Pay

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
