About the role
Medlivo Therapy is seeking a travel Home Health Physical Therapist for a 13-week contract in Seaside, Oregon.
Responsibilities
- Provide physical therapy services in patients' homes
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans as necessary
- Document patient care and treatment sessions accurately
Requirements
- 1 year of recent experience
- Active Physical Therapist License
- Current BLS/BCLS Certification
- Bachelor’s or Master’s Degree in Physical Therapy from an accredited PT program
Qualifications
- Must be able to work flexible hours including evenings and weekends
- Must have reliable transportation to travel to patients’ homes
- Must be able to pass a background check

Pay
Weekly Gross: $2,880.00
