Travel Physical Therapist Home Health - $2,880 per week

About the role

As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Qualifications

• ALL APPLICANTS MUST HAVE 1 year Certified Home Care experience
• 1 year Case Management Experience
• Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement
• Ability to travel to cases as assigned

Schedule

• 13 weeks duration
• 40 hours per week
• 8 hours, days shift
• Weekend rotations are once every 6 weeks
