About the Role

American Traveler is seeking a Home Health Physical Therapist for a 13-week travel assignment in the Albuquerque, NM area. This role serves patients within a 45-mile radius of Albuquerque, including Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen.

Schedule is Monday through Friday, 5x8hr shifts (40 hours per week) with 30 guaranteed hours. The assignment starts on August 31, 2026, and includes a quick start.

Responsibilities

  • Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare, state, and agency standards.
  • Develop, implement, and update individualized plans of care aligned with physician orders and patient needs.
  • Deliver skilled interventions and maintenance therapy programs within state-defined scope of practice.
  • Directly supervise Physical Therapist Assistants and Home Health Aides in compliance with state practice acts.
  • Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.
  • Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies.
  • Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes.
  • Apply ICD-10 coding for accurate and compliant documentation.
  • Present periodic in-service programs to agency staff and contribute to staff education initiatives.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Work independently with minimal daily oversight in patients' homes and community settings.
  • Maintain compliance with HIPAA and Medicare/Medicaid Conditions of Participation.

Requirements

  • Active PT license in the state of practice (New Mexico).
  • Graduate of an accredited physical therapist program approved by CAPTE, APTA, or CAHEA.
  • Two years of clinical experience preferred; home health experience a plus but not required.
  • Current CPR certification.
  • Reliable transportation, valid driver's license, and auto insurance required for home visits.
  • Basic computer proficiency with willingness to learn EMR and OASIS documentation systems.

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined. Mileage is not reimbursed separately, as rates are all-inclusive.
