About the Role
American Traveler is seeking a Physical Therapist for a 16-week travel assignment in a home health setting covering over 70 towns across Vermont and New Hampshire, including White River Junction, VT, Lebanon, NH, Bradford, VT, and Windsor, VT. This full-time, day-shift role involves working 40 hours per week with an average patient visit length of 45 minutes. You will work independently while collaborating closely with an interdisciplinary patient-care team.
Responsibilities
- Evaluate patients' needs for physical therapy and develop individualized treatment plans.
- Carry out treatment programs in accordance with agency policies and professional standards of care.
- Communicate and coordinate with all members of the interdisciplinary healthcare team.
- Document evaluations, treatment goals, and objectives.
- Educate patients and family members on treatment rationale and procedures.
- Build meaningful, ongoing relationships with patients as an integral part of their care team.
Requirements
- Bachelor of Science in Physical Therapy required.
- Active PT license in Vermont and New Hampshire, or an unencumbered compact license.
- Minimum of 1 year of experience as a Physical Therapist in a home healthcare setting.
- BLS certification required.
- Rehabilitation certifications such as ASHA, CHT, or ACP training may be required.
Schedule
- 16-week assignment starting September 7, 2026.
- Full-time, day shift: 5 days per week, 8 hours per day (40 hours per week).
