Responsibilities
- Help patients recover from injuries and manage chronic conditions
- Conduct start of care and revisits
- Work within the coverage territory including Northwest Philadelphia and Montgomery County
Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is required to start the assignment
- BLS Certification may be required from AHA or ARC
- Recent relevant home health experience
- EPIC experience
- OASIS required
