Travel Physical Therapist Home Health - $2,535 per week

Travel Physical Therapist position in home health serving West Portland, OR and surrounding areas. The role involves providing outpatient physical therapy services, conducting patient assessments, developing treatment plans, and implementing therapy in a home care setting. The therapist will adhere to clinical protocols, collaborate with a multidisciplinary team, and support patient recovery and independence.

Responsibilities

• Perform patient assessments and evaluate functional status
• Develop individualized treatment plans
• Implement therapy interventions and monitor progress
• Provide outpatient physical therapy services in patients’ homes
• Adhere to established clinical protocols and documentation standards (Oasis, Homecare Homebase)
• Collaborate with physicians, nurses, and other healthcare team members
• Support patient independence and recovery goals

Qualifications

• Valid Oregon Physical Therapist license
• BCLS certification
• Minimum 1 year of skilled home health experience
• Proficiency with Homecare Homebase (HCHB) software
• Experience with Oasis documentation
• Active driver’s license and reliable transportation
• Ability to attend Monday orientation

Schedule
