Travel Physical Therapist - Home Health - $2,535 per week

About the role

We are seeking a licensed Physical Therapist (PT) to provide skilled home health services to patients in their homes. This role focuses on improving mobility, safety, and independence while supporting patients in their natural living environment.

Responsibilities

• Perform in-home evaluations and develop individualized plans of care
• Provide skilled physical therapy treatments focused on mobility and safety
• Educate patients and caregivers on exercises and fall prevention
• Carefully coordinate care with physicians and interdisciplinary teams
• Complete accurate and timely documentation

Requirements

• Doctorate or Master’s Degree in Physical Therapy
• Active (or pending) state PT license
• Home Health experience preferred, not required
• Strong time management and communication skills

Qualifications

• New graduate PTs may be considered with appropriate support

Skills

• Skilled in providing in-home physical therapy services
• Ability to communicate effectively with patients and caregivers
• Strong organizational and time management skills

Pay

Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
