Responsibilities

• Assess patient conditions in a home health setting.
• Develop individualized care plans to enhance mobility and independence.
• Collaborate with interdisciplinary teams.
• Perform standard therapy measurement techniques such as range of motion, strength, and balance tests.

Schedule

• Days shift, 8 hours per day.
• Minimum of 40 hours per week.

Requirements

• Vermont physical therapist licensure.
• Home health experience.
• OASIS experience.
• McKesson/Netsmart electronic medical record experience strongly preferred.
• No requested time off during the assignment.
• Must be located in Vermont.
