About the Role
Join a respected academic healthcare system through The Ohio State University Wexner Medical Center Home Health program in Columbus, Ohio. This Home Health Physical Therapist opportunity offers the chance to work independently while helping patients regain mobility, improve function, and safely remain in their homes throughout the Columbus area.
You'll provide skilled therapy services to adult and geriatric patients while collaborating with physicians, nursing staff, and interdisciplinary team members to achieve the best possible outcomes.
Position Highlights

Responsibilities
- Perform comprehensive physical therapy evaluations
- Develop and implement individualized treatment plans
- Improve mobility, balance, strength, endurance, and safety
- Provide patient and caregiver education
- Assess home environments and recommend equipment as needed
- Coordinate care with physicians and interdisciplinary team members
- Complete timely and accurate documentation

- Post-hospitalization rehabilitation
Requirements
- Home Health experience preferred
- OASIS experience preferred
- Reliable transportation required
- Current PT license required
- BLS certification required
