Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on each patient's functional goals.
- Treat patients with orthopedic, neurological, post-surgical, and medically complex conditions in the home setting.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
- Educate patients and caregivers on home exercise programs and safety strategies.
- Complete timely EMR documentation and progress reporting.
Qualifications
- Alabama PT License Required
- BLS Required
- Home Health experience preferred
- OASIS and HCHB experience preferred
