About the role

Physical Therapist (PT) specialized in Home Health (HH) services is needed for a position located on the Leeward side of the island. The role involves planning and conducting segments of treatment programs aimed at restoring and improving patients' physical, social, and mental functions. The PT will serve as a co-leader within an interdisciplinary team and will perform comprehensive evaluations, establish individualized treatment plans, and document clinical activities accurately. This position requires active collaboration with patients and team members to support quality care delivery.

Responsibilities

• Plan and conduct segments of treatment programs to restore and improve patients' physical, social, and mental functions
• Serve as a co-leader within an interdisciplinary team
• Perform comprehensive evaluations and establish individualized treatment plans
• Document clinical activities accurately
• Collaborate with patients and team members to support quality care delivery

Requirements

• Bachelor’s degree in Physical Therapy
• Minimum 2 years of physical therapy experience
• Active Hawaii state physical therapist license
• Epic electronic health record experience
• Travel experience
• Current CPR and BLS (AHA) certifications
• Valid driver’s license and current auto insurance
• Must have own transportation
• Prepared for license procurement procedures, including in-person presence at the Hawaii Nursing Board

Preferred Qualifications

• Master’s degree in Physical Therapy
• 1 year of physical therapy home health experience

Schedule

• Start Date: 09/08/2026
• Duration: 13 weeks
• 40 hours per week
• Shift: 8 hours, days

Pay

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined. Mileage reimbursement at $0.43 per mile.
