Travel Home Health Physical Therapist - $2,455 per week

American Traveler is seeking a Travel Home Health Physical Therapist for a 13‑week assignment in Lewistown, Montana. The position starts on October 26, 2026, requires 36 hours per week (three 12‑hour day shifts), and involves traveling to patient homes and other care settings in a Home Health and Hospice environment.

Responsibilities

• Receive and review physician therapy orders for Home Health and Hospice patients
• Complete comprehensive patient evaluations and establish written treatment plans with short‑term and long‑term goals
• Provide skilled interventions including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
• Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
• Educate patients, caregivers, and family members on treatment plans, home exercise programs, safety measures, and expected outcomes
• Maintain accurate and timely documentation including evaluations, progress notes, discharge summaries, recertifications, and resumptions of care
• Collaborate and communicate with physicians, nurses, therapists, and other healthcare professionals involved in patient care
• No holiday coverage required

Requirements

• Current PT license in the state of Montana
• Minimum 2 years of experience as a Physical Therapist
• BLS certification
• Valid driver’s license
