Travel Physical Therapist Home Health - $2,423 per week

About the role

We are seeking a dedicated Physical Therapist to join our home health team. The successful candidate will provide high‑quality therapy to patients in their homes, focusing on individualized patient care and rehabilitation.

Responsibilities

• Conduct patient evaluations and create treatment plans.
• Administer therapy interventions to help patients achieve their rehabilitation goals.
• Document patient progress accurately and timely using electronic health records.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Educate patients and their families about the rehabilitation process and home care techniques.

Requirements

• Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
• Minimum of one‑year physical therapy experience in home health or a facility (preferred).

Pay

Pay package is based on 8‑hour shifts and 40 hours per week (subject to confirmation) with a tax‑free stipend amount to be determined. Contract and local rates may vary based on location and applicant residency.

Schedule

Day shift, 5 × 8‑hour days (08:00 – 17:00), 40 hours per week. Contract length: 13 weeks starting 10/12/2026.
