American Traveler is seeking a Physical Therapist with home health experience for a 13-week travel contract in a home health setting, requiring Homecare Homebase (HCHB) proficiency.

Assignment Overview

• Shift: Days, 5x8hrs
• Holiday rotation is required
• Schedule: Monday–Friday, 8am–5pm with rotating weekend shifts (8am–5pm)

Details

Home health care setting serving patients in their homes across an assigned coverage area
Patient population includes CHF, orthopedic, and pneumonia diagnoses
Daily point expectation of 6.5 points, with responsibilities including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and routine visits
Charting is performed using Homecare Homebase (HCHB)

Requirements

• Minimum 1 year of physical therapy experience required; new graduates are discouraged from applying
• Home health experience is required
• Proficiency with Homecare Homebase (HCHB) is required
• Active BLS certification (AHA) required; pending certifications will not be accepted
• Active state or compact (NLC) PT license required for the state of placement; pending licenses will not be accepted — license required if applicable
• Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and eligibility for rehire
• A copy of a valid driver's license is required for consideration

Additional Information

• Responsibilities include conducting SOC, ROC, recertification, evaluation, and routine home visits
• A clinical manager or educator is available 24/7 for field emergencies and 8am–5pm for general support
• Orientation includes approximately 2 weeks with a work coach, including supervised field visits; orientation totals 12 hours
• Either scrubs or business casual attire is acceptable
• A phone and tablet will be provided for use during the assignment
• Profiles are sent directly to the manager for a phone interview; no clinical team review involved
• Local candidates are accepted for this position
