Travel Physical Therapist General Job

12-week travel assignment in a home healthcare setting starting 08/05/2026.

About the role

Deliver skilled physical therapy services in patients’ homes and collaborate with the interdisciplinary care team. Shifts are 8:30 am–5:00 pm, Monday through Friday.

Responsibilities

• Assess patients in the home setting to determine physical therapy needs and goals
• Develop and implement individualized treatment plans with measurable outcomes
• Deliver PT interventions, including therapeutic exercises, manual therapy, and gait training
• Monitor progress, adjust plans as needed, and document results in patient records
• Educate patients and families on home exercise programs, safety, and fall prevention
• Communicate and collaborate with the interdisciplinary care team to ensure coordinated care

Requirements

• Active Physical Therapist license in Pennsylvania or eligibility for licensure
• Doctor of Physical Therapy (DPT) degree or equivalent
• Ability to provide patient-centered care in a home health setting
• Strong communication and documentation skills
• Ability to travel to patient homes within the service area as required

Pay

Weekly pay range: $2,260–$2,407 with 40 guaranteed hours.

Schedule

8:30 am–5:00 pm, Monday–Friday (5×8) for the 12-week contract.
