Responsibilities
- Assess patients’ physical therapy needs and develop individualized treatment plans
- Implement and adjust rehabilitation programs to promote functional recovery
- Monitor progress, document outcomes, and modify goals as appropriate
- Collaborate with an interdisciplinary team to coordinate care and discharge planning
- Educate patients and families on home exercise programs, safety, and self-management
- Ensure safe patient handling, and adherence to infection control and charting standards
