Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans for residents
• Deliver therapeutic interventions including therapeutic exercises, gait and balance training, mobility assistance, and modalities as appropriate
• Monitor progress, document outcomes, and adjust plans of care to maximize function and maintain safety
• Collaborate with the interdisciplinary care team; communicate resident status, goals, and discharge plans; provide education to residents and families as needed
• Maintain accurate, timely documentation in medical records and ensure compliance with regulatory standards and facility policies

Requirements

• Active Physical Therapist license in South Dakota or eligibility for licensure
• Ability to work effectively in a long-term care setting with geriatric residents
• Strong documentation, communication, and collaboration skills
• Availability for a daytime schedule, Monday through Friday
• Demonstrated commitment to patient safety, professional ethics, and regulatory compliance
