Responsibilities

  • Perform PT evaluations and treatments for a mix of inpatient and outpatient patients
  • Develop and implement individualized treatment plans, with mobility progression from acute care through SNF, and discharge planning
  • Manage 30–45 minute outpatient treatment slots and one-hour initial evaluations; see a daily caseload of 5–15 patients as appropriate
  • Provide care across ER, ICU, Med/Surg (Acute, Observation, SNF) and occasionally OB; contribute to a hospital-based outpatient PT clinic
  • Document patient progress, adjust plans of care, and communicate with the care team to optimize outcomes and safety
  • Support discharge planning through collaboration with physicians, nurses, and therapists, ensuring smooth transitions of care

Requirements

  • Active Hawaii physical therapist license (or eligibility) and ability to obtain license promptly
  • Demonstrated ability to perform PT evaluations and treatments across inpatient and outpatient settings
  • Knowledge of discharge planning and mobility progression
  • Ability to work a 5x8 schedule, including Saturdays as required
  • Comfortable practicing in diverse clinical settings and coordinating across acute and outpatient services
