Responsibilities

• Conduct 1-hour patient evaluations and 45-minute daily treatment sessions
• Develop and implement individualized plans of care for orthopedic, sports, and work-related injuries
• Collaborate with a multidisciplinary rehabilitation team to coordinate patient care
• Document care in EPIC; EPIC experience preferred
• Manage referrals from orthopedic surgeons, family practice providers, and the acute care team
• Monitor patient progress and adapt treatment plans to achieve optimal outcomes

Requirements

• Doctor of Physical Therapy (DPT) or PT degree from an accredited program
• PT licensure: active license or eligibility to obtain licensure as a Physical Therapist per state regulatory requirements
• 1+ year of physical therapy experience; outpatient experience preferred
• Ability to work effectively in an outpatient hospital-based rehabilitation setting
• Strong communication and teamwork skills
• EPIC experience preferred
