Travel Physical Therapist General Job

12-week local home health care travel assignment in NW Philadelphia and Montgomery County, PA. Start date: 09/02/2026.

Responsibilities

• Perform physical therapy evaluations in the home setting and develop a plan of care aligned with patient goals
• Deliver PT interventions per the plan of care, monitor patient progress, and adjust treatment as needed
• Conduct start of care visits and follow-up revisits; document assessments and progress in the designated health record system
• Utilize EPIC for documentation and case management; collect and manage OASIS data as required
• Educate patients and family members on home exercise programs, safety, fall prevention, and activities of daily living
• Collaborate with an interdisciplinary team to coordinate care, communicate progress, and ensure regulatory and quality standards are met

Requirements

• Recent, relevant home health experience
• EPIC experience and OASIS familiarity
• Experience with start of care and revisits
• Ability to fulfill assignments in the NW Philadelphia and Montgomery County area
• Strong communication and documentation skills; dedicated to patient-centered care

Pay

Weekly pay: $2,559–$2,626 per week

Schedule

• Guaranteed hours: 40 per week
• Shift: 8:00am–4:30pm (day shift with consistent schedule)
