Travel Physical Therapist General Job

13-week travel assignment starting 08/31/2026 with guaranteed 36 hours per week.

About the role

Provide one-on-one physical therapy in patients’ home settings within a hospice home-healthcare environment. This temporary travel assignment is through Healthforce Healthcare LLC and includes ongoing credentialing and clinical support.

Responsibilities

• Assess patient condition and develop individualized plans of care to maximize mobility and functional outcomes
• Collaborate with an interdisciplinary team to ensure patient needs are met
• Conduct standard therapy measurements (ROM, strength, balance) and apply a range of modalities
• Perform OASIS-based documentation and case management; maintain accurate EMR records

Requirements

• Active Vermont physical therapy licensure
• Home health experience with OASIS documentation required
• EMR experience preferred (McKesson/Netsmart strongly preferred)
• Ability to travel to patients’ homes within the service area
• Availability for the full 13-week assignment with no planned time off

Pay

Weekly pay range: $2,329–$2,420.

Schedule

Tuesday–Saturday, 8:00 a.m.–5:00 p.m.
