Responsibilities
Conduct comprehensive PT evaluations for residents within the assignment, establishing baseline measures of mobility, strength, balance, gait, pain, and functional independence.
Develop individualized, measurable treatment plans aligned with resident goals and facility protocols; select evidence-based interventions and adjust as progress and safety dictate.
Implement a broad range of therapeutic modalities including therapeutic exercise, gait and balance training, manual therapy, and neuromuscular re-education; document outcomes in the patient record.
Monitor and document resident progress at regular intervals; communicate changes to the interdisciplinary team; participate in care conferences and update plans of care per regulatory requirements.
Collaborate with nursing staff, physicians, social work, and therapy assistants to optimize discharge planning and transition strategies; ensure appropriate referrals and community resources when applicable.
Ensure compliance with infection control, safety standards, documentation accuracy, patient privacy, and regulatory requirements; manage equipment and maintain a clean, safe therapy space.
Requirements
- Doctor of Physical Therapy (DPT) degree from an APTA-accredited program.
- Active physical therapy license to practice in the United States or eligibility for Wyoming licensure.
- Current CPR/BLS certification.
- Experience with geriatric or long-term care populations is preferred.
- Strong written and verbal communication skills; proficient in standard documentation and billing practices.
- Ability to work independently in a travel assignment and adapt to new teams; flexible for a day shift schedule.
