Synergy Medical Staffing is seeking a travel Home Health Physical Therapist for a position in Brunswick, Georgia.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Develop and implement individualized treatment plans based on patient assessment.
• Document patient progress and treatment outcomes accurately.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

• Current licensure to practice as a Physical Therapist in the state of Georgia.
• Bachelor's degree in Physical Therapy from an accredited program.
• At least one year of clinical experience in Home Health setting preferred.
• Passing scores on the National Physical Therapy Examination (NPTE).

Qualifications

• Excellent communication skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.
• Valid driver’s license and reliable transportation.

Skills

• Proficiency in using electronic health records (EHR).
• Knowledge of home health equipment and supplies.
• Effective problem-solving and critical thinking skills.

Pay

The pay package is based on 8-hour shifts and 40.0 hours per week (subject to confirmation), with a tax-free stipend amount to be determined.

Schedule

Shift: 8:00 AM - 16:00 PM, 8.00-5, Duration: 13 weeks.
