Travel Physical Therapist - $2,779 per week

About the role

Wellspring Nurse Source is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 14-week assignment starting on August 3, 2026, in Danvers, Massachusetts.

Responsibilities

• Provide physical therapy services to patients in SNFs
• Develop and implement individualized treatment plans
• Evaluate patient progress and adjust treatment plans as necessary
• Document patient care activities and progress in accordance with regulatory requirements

Requirements

• Current licensure as a Physical Therapist in the state of Massachusetts
• At least one year of recent clinical experience in SNFs
• Bachelor's degree in Physical Therapy from an accredited program
• Passing scores on the National Physical Therapy Examination (NPTE)

Qualifications

• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Strong organizational and time management skills

Skills

• Proficient in using electronic health records (EHRs)
• Knowledge of Medicare and Medicaid regulations
