Required Qualifications
- License: Valid Physical Therapist Professional license in Massachusetts
- Experience: 1 year of experience in a home care setting, 1 year of Certified Home Care experience, 1 year of Case Management experience
Preferred Qualifications
- Experience in hospice, private care, or working as a Case Manager outside of a certified home health agency
Additional Information
- The role requires travel to cases as assigned and weekend rotations approximately once every 6 weeks
- Candidates must demonstrate the ability to collaborate with a multidisciplinary team and maintain thorough, timely electronic documentation in accordance with agency policies and standards of practice
