Opportunity

• Work Monday through Friday, 8:00 AM to 5:00 PM with rotating weekend shifts (8:00 AM to 5:00 PM)
• 5 eight-hour shifts per day schedule
• Complete 6.5 points per day including start of care (SOC), resumption of care (ROC), recertifications, evaluations, and daily visits
• Participate in holiday rotation as required
• Cover home health care setting
• Treat patients with common diagnoses including CHF, orthopedic conditions, and pneumonia
• Clinical manager or educator available 24/7 for emergencies and 8:00 AM to 5:00 PM for general support while in the field
• Two weeks of orientation provided with a work coach and supervised visits
• Chart using Homecare Homebase (HCHB) system
• Phone and tablet provided for use in the field
• Dress code: scrubs or business casual attire
• New graduates not preferred but no strict experience requirement
• Required certification: BLS
• BSN not required
