About the role
The Pharmacist is responsible for the safe, accurate, and efficient distribution of pharmaceutical agents to patients of all ages, hospital employees, and designated areas of the hospital. This role ensures that pharmacy practice is carried out in full compliance with all applicable state and federal regulations, DEA laws, TJC standards, and hospital/departmental policies and procedures. The Pharmacist plays a key role in interdisciplinary patient care by collaborating with physicians, educating healthcare staff and patients, and supporting medication safety initiatives.
Responsibilities
- Accurately review, verify, and dispense medications for patients of all ages in accordance with hospital policies and regulatory standards.
- Collaborate with physicians and clinical teams regarding medication selection, dosing, therapeutic monitoring, and drug interactions.
- Provide medication education and guidance to nursing staff, physicians, and patients.
- Maintain and review patient medication profiles to ensure optimal therapy and patient safety.
- Identify, prevent, document, and report adverse drug reactions and medication errors.
- Support and participate in medication safety initiatives and quality improvement programs.
- Prepare and verify IV admixtures, including nutritional supplements and specialty infusions.
- Ensure proper handling, dispensing, and accountability of controlled substances and narcotics.
- Maintain compliance with DEA regulations, TJC standards, and all applicable state and federal laws.
- Participate in interdisciplinary rounds and clinical discussions as appropriate.
- Maintain accurate pharmacy records and documentation.
Requirements
- Active Arizona Pharmacist License.
- Minimum 3+ years of hospital pharmacy experience in a high-acuity acute care hospital.
- Experience with NICU (Neonatal Intensive Care Unit) patient populations.
- Experience supporting complex cardiac cases, including Impella and ECMO therapies.
- Strong knowledge of IV admixture preparation and sterile compounding.
- Familiarity with medication safety practices and regulatory compliance standards.
- Excellent communication and collaboration skills.
Qualifications
- Advanced clinical training or residency in hospital pharmacy preferred.
- Experience working in a large or tertiary care hospital setting preferred.
