Responsibilities

• Assessing patient physical, psychological, social, spiritual, educational, developmental, and discharge planning needs
• Formulating care plans including discharge planning
• Providing direct patient care, supervision of staff, and delegation
• Documenting care accurately and timely
• Participating in quality improvement activities
• Interacting with all levels of staff, providers, patients, families, and external contacts to coordinate care and support positive patient outcomes
• Maintaining licensure and certifications
• Participating in ongoing education
• Adhering to facility and professional standards
