Occupational Therapist (OT) specializing in Skilled Nursing Facility (SNF) services in Oklahoma. This role involves evaluating and developing individualized care plans for patients with cognitive and Activities of Daily Living (ADL) physical functioning disorders. The occupational therapist will utilize standardized assessments and evidence-based practices to deliver rehabilitative, skilled, and medically necessary treatments aligned with regulatory standards.
Responsibilities
- Screen and evaluate patients
- Develop and implement individualized treatment plans
- Administer procedures such as ADL retraining, therapeutic activities, exercises, modalities, and use of supportive devices
- Document patient progress
- Provide consultation to patients and families
- Participate in clinical meetings
- Supervise therapy assistants
- Adhere to professional practice standards
Requirements
- Active Occupational Therapist license
- 1+ years of occupational therapy experience
- Long-term care (LTC) or rehab experience
- Skills checklist within 1 year
- 0-1 reference at the time of submission
- Must be submitted at the posted rate; if requested rate is higher, candidate will be redirected
