Occupational Therapist, Home Health. This position provides occupational therapy services to geriatric patients in a home health setting, serving approximately 57 patients per day. The role requires flexibility across the assigned territory, timely documentation and communication, and participation in an on-call rotation.

Responsibilities

• Provide occupational therapy services to geriatric patients in a home health setting
• Serve approximately 57 patients per day across assigned territory
• Complete timely documentation and communication
• Participate in on-call rotation

Requirements

• Minimum of 1 year of recent, relevant experience
• Occupational Therapist licensure in hand
• Basic Life Support (BLS) certification
• National Board for Certification in Occupational Therapy (NBCOT) certification
• OASIS experience
• Proficiency in HCHB (Homecare Homebase)
• Demonstrated experience with home health documentation
• Active driver's license and personal transportation required

Schedule

• Day shift with 8-hour shifts
• Minimum 40 hours per week
• On-call rotation may be required every sixth weekend
• Monday orientation scheduled
