Travel Occupational Therapist Home Health - $2,488 per week

About the role

Travel Home Health Occupational Therapist position in West Allis, Wisconsin, covering Kenosha and Racine Counties. 12‑week assignment (13 weeks per facility details) starting 11/02/2026. Full‑time, 40 hours per week, 8‑hour day shifts Monday–Friday. Clinical interview required; manager reviews all offers.

Responsibilities

• Provide occupational therapy evaluation and treatment to adult home‑health patients (geriatrics, post‑stroke, post‑joint replacement, weakness, deconditioning, COPD, CHF, etc.)
• Focus on ADLs and adaptive equipment
• Manage a caseload of 6 patients/“points” per day (evaluation visits = 1.5 points; standard visits = 1 point)
• Participate in morning team huddle via phone each morning
• Collaborate with RN and PT interdisciplinary teams
• Document in EPIC EMR (preferred)
• Travel to patient homes throughout Kenosha and Racine Counties (most visits 20–30 minutes; occasional up to 1 hour)

Requirements

• Minimum 3 years OT experience (SNF, home health, or acute care; home health preferred)
• Active Wisconsin OT license
• BLS certification
• Bachelor’s degree in occupational therapy accepted
• Valid driver’s license, active auto insurance, and reliable vehicle
• First‑time travelers welcome

Qualifications & Skills

• General OT treatment and evaluation skills with emphasis on ADLs and adaptive equipment
• Experience working in interdisciplinary home‑health teams (RN, PT)
• Familiarity with EPIC EMR preferred

Schedule

• Monday–Friday, approximately 07:45–16:00 (shift times around 08:00–16:30 per facility)
• 40 hours per week, 8‑hour shifts
