About the Role
Occupational Therapist Home Health (OT HH) position in Medford, Oregon. This role involves providing skilled occupational therapy services within a home health setting, collaborating with patients to develop and implement individualized treatment plans to improve their functional abilities and quality of life.
Responsibilities
- Provide skilled occupational therapy services in a home health setting.
- Develop and implement individualized treatment plans.
- Manage a patient caseload of up to 6 visits per day, with an emphasis on minimizing drive time.
- Use EPIC/Oasis for electronic charting.
Requirements
- 1 year of skilled home health experience.
- BCLS certification.
- Reliable transportation.
- Oasis certification.
- Eligibility to obtain an Oregon state license.
- Primary series COVID vaccination plus one booster.
Preferred Qualifications
- Experience with electronic charting systems.
- Familiarity with home health care protocols.
Schedule
- 13-week assignment starting 08/31/2026.
- 40 hours per week, 8-hour day shifts.
- No on-call duties or floating between units.

Pay
Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
