Qualifications

• Wisconsin Occupational Therapist license
• BLS certification
• Bachelor's degree in Occupational Therapy
• Valid driver's license and vehicle
• Active auto insurance
• Minimum 3 years occupational therapy experience in home health, skilled nursing facility, or acute care settings
