Responsibilities
- Provide patient care supporting safe transitions, including handoffs and care plan updates.
- Prepare patients for diagnostic tests or procedures and conduct post-procedure monitoring.
- Perform focused reassessments and escalate concerns through appropriate clinical channels.
Qualifications & Skills
- Minimum of 2 years of relevant experience.
- Ability to work 36 hours per week with 12-hour shifts.
