Responsibilities

• Assess patient status and respond to changes according to clinical protocols.
• Document assessments, interventions, and patient progress accurately.
• Support safe care transitions, including handoffs and care plan updates.
• Provide patient and family education on care plans and discharge instructions.

Requirements

• Minimum of 2 years of relevant experience.

Schedule
