Requirements
- Minimum of 2+ years of dialysis experience
Responsibilities
- Perform patient intake, focused assessments, and symptom screening in accordance with clinic protocols
- Coordinate care activities among healthcare providers to ensure continuity of care
- Administer prescribed therapies and monitor patients for adverse reactions or condition changes

- Strong clinical skills in dialysis care
- Ability to support care coordination and patient monitoring
