Responsibilities
- Provide direct patient care under the supervision of an RN and within the scope of practice for an LPN in an LTC setting
- Maintain and monitor residents, document vitals, intake and output, changes in condition, and promptly report to the supervising RN
- Administer prescribed medications and perform routine treatments as allowed by licensure and facility policy
- Absorb and assist with activities of daily living, mobility, hygiene, nutrition, and comfort measures
- Follow individualized care plans, promote safety, fall prevention, and infection control practices
- Document actions in the electronic health record per facility policy and communicate clearly with the health care team
Requirements
- Active LPN license (standard requirement for practice)
- CPR/BLS certification as required by facility policy
- Ability to work in a long-term care setting and collaborate with a multidisciplinary team
- Strong communication, documentation, and time-management skills
- Prior LTC or skilled nursing experience preferred if available
