Travel Licensed Clinical Social Worker (LCSW)

Role and Responsibilities

• Provide discharge planning and psychosocial interventions through assessment, advocacy, referral, and interdisciplinary collaboration.
• Prepare social histories and assess patients' social, emotional, psychological, and financial needs.
• Manage policies for setting and evaluating residents' short and long-term goals.
• Offer social casework to individuals and families in accordance with physicians' plans of care.
• Assist healthcare providers in understanding significant social and emotional factors affecting patients.
• Document services in compliance with regulations and third-party payer guidelines.

Qualifications and Skills

• Valid LCSW credential required.
• Minimum of 2 years of relevant experience.
• Strong decision-making, communication, and interpersonal skills.
• Preferably hold a Master’s degree from a CSWE-accredited school.
• Ability to work independently with attention to detail and patient safety.
