Responsibilities
- Conduct biopsychosocial assessments, develop care plans, and implement interventions
- Coordinate care and discharge planning, utilizing regional, state, and federal resources
- Manage crisis counseling, suicide assessments, and short-term counseling
- Facilitate family meetings and provide end-of-life and advance care planning support
- Work effectively in an oncology setting with seriously ill patients and families
- Utilize Telehealth systems and work in a fast-paced healthcare environment
Qualifications & Skills
- Maryland LCSW-C or equivalent advanced license from out of state
- Minimum hospital experience; oncology or palliative/hospice experience preferred
- Epic system proficiency
- Experience in psychological, social, emotional, financial, and practical care domains
- Knowledge of health insurance and financial implications of chronic illness
- Experience with crisis counseling, grief, and terminal illness support
- Comfort working with seriously ill patients and families in end-of-life situations
- Ability to perform comprehensive assessments and coordinate multiple aspects of patient care
Additional Requirements
- BLS certification
- Pharmacology knowledge
- Skills checklist and specialty exam completion
